Welcome to Expert Pharmacist Review
Our accredited pharmacists have many years of experience in clinical pharmacy. We can help clients understand their medicines, provide up-to-date medicines lists, and working with other healthcare members to improve health and wellbeing of the clients.
Home Medicines Reviews are fully funded by the Australian Government and are of no costs to eligible clients.
We are currently performing Home Medicines Reviews in the inner suburbs of Melbourne, but can also visit people in other areas.
Meet Our Accredited Pharmacists
All our pharmacists are registered with Australian Health Practitioner Regulation Agency (APHRA). In addition, they are also accredited by AACP or SHPA to undertake medication management reviews, such as Home Medicines Reviews (HMRs).
Hoa Tran
Hoa is a Consultant Pharmacist. She is accredited by AACP and has been performing Home Medicines Reviews (HMRs) since 2007.
She has experience in many areas of pharmacy, particularly in geriatric and general medicine. Her qualifications include Bachelor of Pharmacy with Professional Honours, Certified Geriatric Pharmacist (CGP) and SHPA Fellowship.
She is passionate about quality use of medicines, and wants to help people to improve their medication management and be in control of their own health.

No Costs
Your Home Medicines Reviews (HMRs) are fully funded by the Australian Government if you are eligible clients. Click here for eligibility criteria.
Prompt Service
We are commited to provide a prompt service. Once we have received your referral, we will aim to conduct the review and report to the doctor within 14 days.
Easy in-home Interview
Home Medicines Reviews are conducted in the comfort of your home. You can also have a family member, carer, or friend present at the interview if you wish.
Telehealth Option
You can also choose to have your Home Medicines Review conducted via Telehealth for eligible clients. Click here for eligibility criteria..
Follow up Appointments
If necessary, follow-up sessions are available to sort out any unresolved medication-related issues.
