The Home Medicines Review Process
There are 5 stages to the process for pharmacists to obtain a Home Medicines Reviews from an accredited pharmacist for their clients:
1. Identify suitable clients
2. Request a HMR referral from the client's doctor
3. Provide the client's dispensing history
4. Email or fax the HMR referral
5. Review the Medication Management Plan
